What Forms Do I Need To Turn In To My Coach/Advisor? 

Code Card
Every student needs to sign this card for EACH sport, club or activity they participate in.

Concussion Form
Every student needs to sign this form for EACH sport, club or activity they participate in.

Emergency Information Card
Again...you need to turn this form in to your coach/advisor for EACH activity you participate in during the year.

Physical Exam Card or Alternate Year Physical Card
You need to turn in an Initial Physical Card (signed by your physician) to your coach/advisor when you participate in any activity/sport for your first time at Lake Denoon Middle School.  Your physical card will then be good for the remaining years while you are at Lake Denoon Middle School.  If you decided to participate in another sport/activity during the year or any following year, you only are required to complete and turn in the AUXILIARY Form.
