Medication At School

Any medications that will administered at school must be accompanied by a district medication form signed by the prescribing physician and parent if it is  prescription medication

OR

a district medication form signed by a parent if it is  non-prescription (over-the-counter) medication.

Cough Drops and throat lozenges must be presented to the classroom teacher with written istructions such as frequency and dosage.

Muskego Elementary does not have a supply of medication to administer to a student. All medications must be supplied by a parent or guardian.

All medication sent to school must be in the original container or prescription bottle. No medication will be accepted that arrives to school in baggies, envelopes, etc.
